It is best if you Download the Form, Complete it, take a READABLE PIC, use the "Contact Us" Page to upload it and then Save for your own records.  This will be the basis for how we will begin to create a compelling image.

We must receive, and CONFIRM RECEIPT of, your COMPLETED Video Set-Up Form BEFORE the Consultation; ABSOLUTELY NO EXCEPTIONS

Upon completion, schedule an appointment (below the Form) to discuss the particulars so that we can determine if our relationship will be a good fit for the both of us.  We can either do the Consultation In-Person at our offices or Virtually, your choice.
